Can HIFU Make the Face Look Hollow? Areas That Need Conservative Planning

HIFU can make a face look hollow when deep energy is placed in areas that already have little fat or when treatment is more aggressive than the face needs. That does not mean everyone becomes hollow after HIFU. The result depends on location, depth and the treatment goal.
At STC, Ultraformer III is mapped before treatment: where support is needed, where useful cheek volume should be preserved and where no lines are necessary. We do not use the same full-face pattern for every patient [1].
Which areas need the most caution?
The temples, central cheeks and area beneath the cheekbones rely on volume for a rested look. Thin faces, existing hollows and major recent weight loss usually need conservative or no deep treatment in these zones.
The jawline and under-chin area may be more suitable when the actual problem is lax tissue or a blurred contour, although tissue thickness and nerve anatomy still matter.
A refined contour is not the same as hollowing
A good contour result sharpens the jawline while keeping volume where it belongs. Hollowing makes grooves deeper, cheekbones harsher and the face more tired. Both should not be marketed simply as a ‘slimmer face’.
Are more lines automatically better?
No. Line count only makes sense together with cartridge, depth, energy, spacing and treatment zone. STC focuses on putting an appropriate number of lines in the right areas, not chasing the largest number.
Who should be cautious with full-face HIFU?
A thin or already hollow face may need jawline-only treatment or skin-focused RF such as Oligio X when broad loss of firmness is the main concern.
Frequently Asked Questions
Can a thin person have HIFU?
Yes, in selected zones. The plan should support contour rather than reduce facial volume.
Can HIFU reduce cheek fat?
Ultraformer III has different cartridges and depths, but a facial lifting plan should not be treated as indiscriminate fat removal. First confirm that the area would genuinely benefit from volume reduction.
Should I repeat HIFU if my cheeks look hollow?
Do not rush to repeat. Wait for swelling and early changes to settle, then have the face reassessed.
If hollowing is a concern, send STC unfiltered front and side photos. We can assess which zones to lift, preserve or avoid and whether Ultraformer III or Oligio X is the better starting point.
