Heart Diseases and Stem Cell Therapy

โรคหัวใจ คือ โรคหัวใจและหลอดเลือดซึ่งรวมไปถึงโรคความดันโลหิตสูง โรคหลอดเลือดหัวใจ (Coronary Artery Disease: CAD หรือ Coronary Heart Disease: CHD) ก็คือ โรคที่เกิดจากหลอดเลือดหัวใจ ‘Coronary Artery’ ตีบแคบหรือตีบตัน ส่งผลให้กล้ามเนื้อหัวใจขาดเลือดหรือกล้ามเนื้อหัวใจตาย หัวใจจึงทำงานผิดปกติส่งผลถึงอวัยวะต่างๆ ขาดเลือดไปด้วย ทำให้เกิดมีอาการต่างๆ โรคหลอดเลือดหัวใจเป็นโรคของผู้ใหญ่ตั้งแต่วัยหนุ่มสาวไปจนถึงในผู้สูงอายุ

สาเหตุของโรคหลอดเลือดหัวใจเกิดจาก Plaque ซึ่งเป็นไขมันในเลือดไปเกาะตามผนังของหลอดเลือดหัวใจเป็นระยะเวลานาน ทำให้มีการเกาะของเกล็ดเลือด และเม็ดเลือดขาว ส่งผลให้ผนังหลอดเลือดแข็งหนาและตีบแคบ และยังก่อให้เกิดการอักเสบของผนังหลอดเลือดทำให้มีการอุดตันของหลอดเลือดแดง ทำให้เลือดไปหล่อเลี้ยงกล้ามเนื้อหัวใจไม่เพียงพอ จนเกิดเป็น ‘โรคกล้ามเนื้อหัวใจขาดเลือด และกล้ามเนื้อหัวใจตาย (Myocardial Infarction: MI) ในที่สุด’ ซึ่งอาจเกิดได้อย่างเฉียบพลัน และเมื่อรุนแรงจะเป็นสาเหตุให้หัวใจหยุดทำงานทันที จึงเสียชีวิตได้ทันทีกะทันหัน

อาการพบบ่อยของโรคหลอดเลือดหัวใจ เช่น ไม่มีอาการเมื่อเริ่มเป็นโรคหรือเมื่อหลอดเลือดยังตีบไม่มาก เหนื่อยง่ายเมื่อออกแรงหรือออกกำลัง เจ็บแน่นหน้าอกเมื่อใช้กำลังเพิ่มขึ้นหรือเมื่อมีความเครียด อาการอาจร้าวไปที่ขากรรไกร ไหล่ และ/หรือ แขนด้านใดก็ได้ แต่มักเป็นด้านซ้าย อาการของโรคหัวใจล้มเหลว (Heart Failure: HF) เช่น เหนื่อยง่าย หัวใจเต้นเร็ว บวมหน้าแขน/ขา ความดันโลหิตสูง ตรวจเลือดพบไขมันในเลือดสูง

การรักษาโรคหัวใจและหลอดเลือด คือใช้ยาขยายหลอดเลือด ยาต้านแข็งตัวของเลือดเพื่อลดการเกาะตัวของเกล็ดเลือด เช่น Aspirin ยาลดไขมันในเลือดอย่าง Statin ยาลดความดันโลหิต เป็นต้น รวมไปถึงปรับพฤติกรรมการกินและการออกกำลังกายให้สอดคล้องกับโรค

สําหรับบทบาทของสเต็มเซลล์ในการรักษาโรคหัวใจ ได้มีการศึกษาวิจัยโดยนําสเต็มเซลล์มา พัฒนาเป็นเซลล์กล้ามเนื้อหัวใจและหลอดเลือด เพื่อรักษาผู้ป่วย หรือโรคทางระบบหลอดเลือดหัวใจเป็นต้น การรักษาด้วยการปลูกถ่ายหัวใจสามารถทำให้คุณภาพชีวิตของผู้ป่วยดีขึ้นได้ แต่มีข้อจำกัดคือการขาดแคลนผู้บริจาคอวัยวะ ด้วยเหตุนี้จึงมีความพยายามที่จะนำเซลล์ต้นกำเนิดมาใช้ในการรักษา โดยมีความคาดหวังว่าเซลล์ต้นกำเนิดจะสามารถซ่อมแซมเซลล์ส่วนที่เสียหาย ยับยั้งการดำเนินของโรค และเพิ่มประสิทธิภาพการทำงานของหัวใจได้ มีข้อมูลเกี่ยวกับการศึกษาวิจัยการนำเซลล์ต้นกำเนิดมาใช้ในการรักษาโรคกล้ามเนื้อหัวใจตาย จนถึงปัจจุบันมีการทดลองทางคลินิกจำนวนมาก ซึ่งมีการใช้สเต็มเซลล์และวิธีการให้แตกต่างกัน โดยชนิดของเซลล์ต้นกำเนิดที่มีการศึกษา ได้แก่ Bone Marrow-Derived Stem Cells, Skeletal Myoblasts, Cardiac Progenitor Cells, Embryonic Stem Cells และ MSCs จาก Bone Marrow หรือ Umbilical Cord มีการให้เซลล์ต้นกำเนิดด้วยวิธีการต่างๆ ได้แก่ การให้ทางหลอดเลือดดำ การให้ทางหลอดเลือดหัวใจ และการฉีดเข้าผนังกล้ามเนื้อหัวใจโดยตรง ผลการศึกษาส่วนใหญ่พบว่า การรักษาด้วยเซลล์ต้นกำเนิดค่อนข้างปลอดภัย แต่ประสิทธิภาพของการรักษา ยังไม่สอดคล้องกันในการศึกษาต่างๆ ทำให้ยังไม่ได้ข้อสรุปที่แน่ชัด ในรายที่ได้ผลดีจากการรักษาก็ยังไม่ชัดเจนว่าเกิดจากการที่เซลล์ต้นกำเนิดเปลี่ยนไปเป็นเซลล์กล้ามเนื้อหัวใจ หรือเกิดจากสารบางชนิดที่หลั่งจากเซลล์ต้นกำเนิดไปมีผลทั้งทางตรงและทางอ้อมในการเพิ่มประสิทธิภาพการทำงานของหัวใจ ล่าสุดมีรายงานการวิจัยที่ใช้ Allo-MSCs ในกล้ามเนื้อหัวใจตายเฉียบพลัน และ โรคหัวใจวาย พบว่ามีความปลอดภัยและเพิ่มประสิทธิภาพการทำงานของหัวใจ มีการเพิ่มเปอร์เซ็นต์ของเลือดที่ถูกบีบออกจากหัวใจต่อการบีบตัวของหัวใจ 1 ครั้ง (Eject Fraction) และ การทำงานของ Left Ventricular Function ดีขึ้น องค์การอาหารและยาของอเมริกา (FDA) ได้อนุมัติการใช้ Allo- MSCs ในการรักษาป้องกัน ภาวะหัวใจวายที่เป็นผลจาก ภาวะกล้ามเนื้อหัวใจตายใน Clinical Trial Phase I โดยบริษัท Osiris Therauptics Inc. ซึ่งพบว่าผู้ป่วย 53 ราย ได้รับ Allogeneic Mesenchymal Stem Cells (Allo-MSCs) ทางหลอดเลือดดำภายใน 7 วันหลังมีอาการกล้ามเนื้อหัวใจตายในครั้งแรก โดยผู้ป่วยถูกติดตามอาการไปถึง 2 ปี ผลการศึกษาพบว่าทำให้การบีบเลือดออกจากหัวใจได้มากขึ้น และการทำงานของหัวใจดีขึ้น คุณภาพชีวิตของคนไข้โรคหัวใจดีขึ้น



รูปที่ 2 ผลการศึกษา Preclinical (A) และ Clinical Trial (B) พบว่าสเต็มเซลล์ทำให้กล้ามเนื้อหัวใจมีความหนาขึ้น เมื่อเปรียบเทียบกับกลุ่มควบคุม และมีผลทำให้หัวใจบีบเลือดออกจากหัวใจ (% Left Ventricular Ejection Fraction) ได้มากขึ้น การทำงานของหัวใจดีขึ้น (Chen et al., 2004; Ge et al., 2006)

กลไกการทำงานของ MSCs เมื่อเข้าไปสู่ร่างกายทางหลอดเลือดดำ MSCs จะกระจายไปตามร่างกายไปกระตุ้นการทำงานของหัวใจ โดยกระตุ้นการทำงานของ Endothelial Cells และ Local Stem Cells ทำให้การทำงานของหลอดเลือดดีขึ้น ลดการอักเสบ เพิ่มความหนาแน่นของกล้ามเนื้อ นอกจากนี้ยังมี Paracrine Effect จาก MSCs ที่หลั่งสาร Soluble Factors ซึ่งเป็น Active Molecules ได้แก่ TGFb VEGF SDF-1 และ EGF ซึ่งไปกระตุ้นการทำงานของ Local Stem Cells ทำให้มีการกระตุ้นการซ่อมแซมเนื้อเยื่อกล้ามเนื้อหัวใจได้ดีขึ้น การหลั่ง Cytokine จาก MSCs สามารถกระตุ้นการซ่อมแซมเซลล์และเนื้อเยื่อหัวใจ (Regeneration) และกระตุ้นการสร้างหลอดเลือดใหม่ (Neovasculalization) ลดการตายของเซลล์เนื้อเยื่อหัวใจ กระตุ้นการทำงานการเพิ่มจำนวน Endogenous Stem Cells เพื่อซ่อมแซมและทดแทนเซลล์ที่ตายไป
